Welcome to LPCN News May 2024

The latest LPCN bulletin is here, with project updates, new training dates, an update on the LPCN website and our new patient and carer leaflet which has been well received throughout the palliative and end of life care community.

Whilst we all continue to face significant financial challenges, we’d like to thank all our partners and colleagues for your continued enthusiasm and support for our work throughout Leeds. Watch this space!

Planning Ahead Update

We are pleased to report that all the training that has taken place around Planning Ahead / Advance Care Planning is showing an increase in the number of people who have a ReSPECT plan in place.

Well done to everyone involved!

Transfer of Care Group Update

The LPCN Transfer of Care Group is a good example of a collaborative approach to improving patient experience and streamlining care.  LTHT, St Gemma's Hospice and Sue Ryder Wheatfield's Hospice have implemented a cost improvement initiative to reduce medication waste.

We were aware that patients transferring to Leeds Hospices were being discharged with medication that was often wasted because of changes in condition requiring a different clinical approach. In an attempt to reduce waste (both financial costs and staff time) we agreed that not all medication should be sent.

The success of this project has a clear waste reduction impact and is a good example of collaborative and innovative partnership working.

We have spaces available on the next sessions of Planning Ahead Training and Palliative Care for the Homeless and Vulnerably Housed. Dates and how to book can be found on our website here.
New LPCN leaflet now available

We have recently published a revamped version of our popular End of Life Care and Support in Leeds leaflet. It can be downloaded from our website here. Hard copies will be distributed to key sites over the next few weeks.

Dying Matters Week 2024

Dying Matters Week 2024 takes place from 6-12 May. A new exhibition is being launched at Leeds City Museum to mark the occasion – click here for more information. There’s also lots of national update and information on the Hospice UK website here.
