Leeds Community Healthcare NHS Trust End of Life Care Facilitators

Leeds Community Healthcare NHS Trust End of Life Care Facilitators are a team of senior nurses who offer clinical support, education and training to staff caring for residents who are approaching the end of their life in a care home. Training can be arranged within care homes on a range of topics. 

The courses are free and include: 

  • Verification of Expected Death
  • Syringe Driver Management 
  • Symptom Management 
  • Challenging Communication at the End of Life
  • Planning Ahead/Advance Care Planning
  • Care in the Last Days of Life

Leeds City Council (Care Quality Team)

The Care Quality Team is part of Leeds City Council, and works with care homes and home care in Leeds, providing information, advice, guidance and support.  The team will be facilitating information and quality improvement events on the following dates:

  • Wednesday 10th September 14:00 to 15:30pm
  • Wednesday 12th November 14:00 to 15:30pm
